When you visit a hospital, the care you receive involves multiple medical staff members that prescribe a series of tests to evaluate your illness or injuries, run medical interventions to restore your health on the long run, and monitor your condition and attend to you until you can return home. A positive outcome requires a well-orchestrated plan where everyone involved knows your health condition, your most important needs, your health risks, your previous test results, and who is next in line to take care of you. Continuum of care is the system that provides continuity of the medical care delivered to you, especially when switching between physicians, nurses, and medical institutions. Health Informatics plays an essential role in creating a framework for a high quality continuum of care. Our speaker, Gary Hutchinson will describe the advantages and challenges of health informatics and how they impact the continuum of care.
